Types of FTM Top Surgery

Transgender top surgery, FTM surgery, can be an essential and life-changing event in many patients’ lives.

Also known as gender-confirming surgery, female-to-male top surgery can include procedures such as:

  • Chest or breast reduction surgery
  • Transmasculine top surgery or a subcutaneous mastectomy
  • Male chest contouring
  • Cosmetic mastectomy

In this article, expert FTM procedural surgeon John Zinsser MD explores FTM surgery, what the procedures achieve, the different types of FTM surgery, and how you can either begin your FTM journey or help a loved one on theirs.

What Does FTM Surgery Achieve?

FTM surgery helps patients overcome the body dysmorphia associated with transness. For trans men, breasts can be among the most prominent physical features that need addressing.

FTM surgeries are procedures that eliminate breast tissue and sculpt the chest into a more masculine aesthetic.

What’s the Difference Between FTM Surgery and Breast Reduction?

While the principles of an FTM surgery for trans men remain the same as a breast reduction for a cisgender woman, the procedure’s goals and desired outcomes are different.

Breast reduction aims at maintaining a feminine form, but with a reduced amount of breast tissue, so patients can relieve physical strain on their frame and more freely enjoy a more active lifestyle.

On the other hand, FTM procedures generally aim to completely reform the chest and torso to appear more masculine, reflecting your true identity. As such, those seeking FTM surgery must consult a plastic surgeon with specific expertise in consulting and guiding trans people through gender-confirming surgery, instead of those who have the technical capacity for performing breast reductions.

Different Types of FTM Surgery

FTM surgeries offer various surgical approaches. The primary difference revolves around the type of incision, depending on your initial chest and bust and your desired outcome.

  • Double incision: Using liposuction to remove fatty tissue, Dr. Zinsser makes incisions above and beneath the pectoral muscle, completely removing the nipple, which he will later reattach after removing the desired amount of fat.
  • Buttonhole incision: This pattern is similar to the double incision, but leaves the nipple completely intact (or possibly modified for size).
  • Inverted-T incision: This incision approaches the nipple and areola from a different location.
  • Peri-areolar incision: Dr. Zinsser may recommend this FTM procedure if you have a smaller initial bust size. First, he makes an incision around the border of the areola, then creates a second, larger concentric circle around the first incisions and removes the resulting ring of skin. He may perform additional contouring with liposuction before bringing the remaining skin together.
